Sometimes waiting yields better outcomes. Repair of an orbital floor fracture involves bridging of the floor defect using one of the various biomaterials. More commonly, titanium meshes, porous polyethylene sheets, or autologous bone grafts. Titanium meshes and bone grafts are radiopaque. Orbital floor fracture repair might be indicated in this setting for small or medium sized defects. Appropriate timing is based on the clinical exam and imaging. Oculocardiac reflex — bradycardia and nausea from traction on orbital contents with significant herniation of orbital contents; can be without clinical evidence of extraocular muscle The challenge of orbital floor repair is particular in the most posterior part and in the transition zone with the medial wall.

This exhibit depicts the surgical repair of right orbital floor and medial wall fractures. Pre-operatively, severe orbital floor and medial wall fractures can be seen

Tripod fractures can also be associated with orbital floor fractures but with a different mechanism. METHODS: A retrospective comparative case series of all orbital floor fracture repair cases (158 eyes) from 1983 to 1998 was done. Synthetic prostheses were tailored to the orbital floor in clinically significant fractures. Bone grafts were chosen for large defects, comminuted fractures or if other reconstruction (eg, sinus) was required.

This surgery cannot repair all problems associated with trauma to the face. It is important to note that some patients have unrealistic expectations about how orbital surgery will impact their lives.

According to Kim and Jeong (2016), isolated orbital fractures are encountered in 4-16% of all facial fractures, and orbital fractures composed approximately 30-55% of the zygomaticomaxillary complex (ZMC) and naso-orbital-ethmoid (NOE) fractures. The ideal material for orbital floor fracture repair is one that is resorbable, osteoconductive, resistant to infection, minimally reactive, does

In one patient trauma preceded the onset of proptosis. In all three cases, the cause of the proptosis was probably hemorrhage of the capillaries in a fibrous capsule that surrounded the implant and that was evident on orbital CT scan. Timing of return to normal activities after orbital floor fracture repair Orbital floor strength is regained 24 days after repair. The authors now let patients resume normal activities approximately 3 weeks after uncomplicated orbital floor fracture repair. This is one of many clinical factors in assessing the return to normal activities. Repair is typically via a transconjunctival approach with complete reduction of the orbital contents from the fracture site and placement of an alloplastic implant.

Jul 17, 2014 Fractures of the orbital floor are commonly repaired by all specialists who manage facial trauma.
